• Pro Cure致力保障病人私隱權。透過此表格收集閣下之資料，只用於作直接幫助閣下醫療,預約或查詢之用。閣下的個人資料將會被保密，未經閣下同意，本中心將不會向第三者披露。 但在法律要求下，本中心可能需披露閣下的個人資料。
• 如未能出示或提供身份證明文件, 本診所有權不提供任何證明文件(如病假證明信,到診證明信, 轉介信及醫療報告等。
•根據《個人資料私隱條例》，如閣下欲查閱或更改個人資料，可致函香港天后永興街4號福園一樓全層，作書面要求。
•本人同意及知悉以上私隱政策聲明及免責聲明。

•Pro Cure is committed to protecting the privacy of patients. The information collected from you through this form will be used directly with medical treatment, appointments or enquiries. Your personal data will be kept confidential and the center will not disclose it to third parties without your consent. However, if required by law, the center may need to disclose your personal data.
• Our clinic may discretion reject the consultation and DO NOT provide any medical documents (eg: sickleave letter, attendance letter, referral letter and medical report, etc.)if fails to show the identification document (e.g. Hong Kong Identity Card) .
•According to the "Personal Data Privacy Ordinance", if you wish to access or change your personal data, you may notify the company.
